JOB SUMMARY:     Primary responsibility for the care and management of the clients enrolled in the Community Paramedicine Program.  Responsibility includes but is not limited to assessment, interaction, and treatment of those clients enrolled in the Community Paramedicine Program, including facilitating continuity of care by interacting with other members of their interdisciplinary team; Also responsible for participating in advanced medical research and treatment modalities as directed by the Medical Director; and other operational support functions as assigned. Additional duties include leadership roles and training/mentoring as assigned.

REPORTS TO:         Mobile Healthcare Coordinator

EQUIPMENT: Computer, Printer, Copier, Fax, Phone, ipad, LP15, Blood Pressure cuff and sphygmomanometer, glucometer, 700mH radio

  1. Perform physical and home assessments based on the protocol for the patient.
  2. Function in non-traditional settings and provide non-emergency care with a long term focus.
  3. Conducting environmental assessments in support of patient’s plan of care; identify items/actions that contradict the plan of care.
  4. Provide ongoing interaction with patients in the form of telephone calls and home visits.
  5. Provide disease specific education to the patient and/or caregivers in verbal and written formats.
  6. Regularly communicate with multiple agencies and other members of the Community Paramedicine team to facilitate continuity of care objectives to ensure that the patient is receiving all available services and proper follow up.  Examples of these agencies are the primary care physician, hospital case managers, social services, area council on agencies and other community service organizations.
  7. Maintain accurate and current medical records on the patient utilizing the Health EMS platform.
  8. Multi-task and make sound decisions in critical situations.
  9. Research and maintain list of all services available to the patient such as United Way, clinics that accept the patient’s insurance, Meals on Wheels, VOA, etc.
  10. Participates in activities such as blood pressure or immunization clinics that promote the Community Paramedicine division and the Acadian organization.
  11. Performs other duties as assigned by the Clinical Services Administrator, Medical Director and Acadian Companies.

 

QUALIFICATIONS:

  1. High school diploma or GED equivalent
  2. Current CPR and ACLS certifications
  3. Maintain national and state certification at the level of EMT-P within the state where services are to be provided
  4. Strong and effective verbal, written, and interpersonal communication skills.
  5. Demonstrated ability to provide effective coaching and leadership.
  6. Demonstrated teaching and educational facilitation skills
  7. Ability to adhere to established standards for educational quality
  8. Familiarity with basic computer applications and functions
  9. Recommendation from their current OM, QIC, VP and/or Medical Director
  10. No significant clinical issues in the past one (1) year
  11. Minimum of two (2) years of experience as a Paramedic
  12. Must be able to function as a field paramedic.
  13. Team oriented and able to communicate and work effectively and efficiently with others
  14. Familiarity with documentation software including applicable paperwork.
  15. Valid driver’s license and acceptable driving record in accordance with Acadian’s driving and insurability policies.
  16. PREFERRED REQUIREMENTS:
  17. Associate’s degree in emergency medical services management, business administration, or other related degree, or equivalent experience
  18. Previous trainer or instructor experience
  19. Previous Critical Care experience
